At Premier Wound Care of TN(PWC), we are deeply honored by the confidence placed in our team by the physicians and caregivers who refer patients to us for in-home wound care services. We recognize the responsibility that comes with your trust and remain committed to delivering compassionate, expert care to your patients in the comfort of their homes.

What to Expect After Submitting a Referral
Upon receipt of your referral, our team promptly initiates a thorough review to ensure that all necessary documentation is complete. This includes verification of patient demographics, insurance information, and clinical records.
We work directly with insurance providers to secure all required pre-authorizations in advance. If any additional information is needed, we will reach out to your office.
Once insurance authorization is obtained and all documentation is verified, our team will contact the patient directly to schedule an in-home visit with one of our certified wound care nurse practitioners. Our medical scheduler will reach out to your office with the date of the patient’s appointment.
Following the visit, a detailed medical report from our nurse practitioner will be provided to your office within 3 to 5 business days.

What is needed for a referral?
To begin treatment as soon as possible the following information is necessary. Any missing information may delay patient care.
- Patient demographic sheet.
- If the referring office has multiple phone numbers for the patient, please include each number.
- Patient insurance card(s).
- Referral diagnosis/description of why the patient is being referred.
- Last office note the medication and allergy list.
- Most recent labs related to nutrition, diabetes, and if available, wound cultures.
- Most recent imaging reports – radiology reports or CT scans.
- Please list the referring office name, phone, and fax number and the name of the individual sending the referral.
